Compassionate, Culturally-Sensitive Care

Our mission is to provide compassionate, culturally-sensitive care and services to help our participants remain healthy, independent, and safe in the community, thus improving their quality-of-life while reducing their need for frequent hospitalizations and long-term placement.

 SKILLED NURSING SERVICES

Registered and Licensed Practical Nurses at Rainbow ADHC are able to provide the full range of skilled nursing in accordance with your physicians’ orders.  Our focus is on preventative care to proactively troubleshoot any symptoms and adverse responses to medications or treatments as quickly as they present themselves.  We maintain close communication with your Primary Care Provider so that your interdisciplinary team works in unison to help achieve your goal of thriving safely in the community.

  • Skilled observation and complete care assessments designed specifically for you by our interdisciplinary team
  • Wound care for chronic, non-healing wounds due to diabetes, chronic circulatory deficiencies, pressure ulcers, and wounds resulting from surgical and other medical treatments
  • Medication management and teaching
    Bowel/bladder training
  • Education about diabetic care and daily Blood Sugar monitoring
  • Foley catheter insertion and teaching care of in-dwelling foley catheter
  • Administer vitamin B12, insulin, and other IM/Subq. medications
  • Teach -ostomy or -ileo conduit care
  • Teach gastrostomy feeding and parenteral nutrition
  • Administer inhalation Rx
  • Daily monitoring of vital signs
  • Personal care assistance with showering, dressing, and all other activities of daily living

THERAPEUTIC ACTIVITIES

Rainbow ADHC Centers offer recreational and therapeutic activities that are tailored to your individual needs and designed to help improve or maintain your level of independence and wellness.  We offer activities that are cognitively stimulating, help enhance your memory, promote your physical fitness, and create opportunities for your unique self-expression.

Rainbow ADHC’s interdisciplinary teams design therapeutic activities with your input to meet your specific needs and interests.  We’ll help you improve your strength and range-of-motion, your memory and cognition, and your self-awareness and independence.

Participant Advisory Councils meet regularly to provide feedback regarding scheduled activities, which include:

  • Daily morning exercises, and additional Tai Chi, yoga, and dance classes
  • Monthly outings to local theaters, restaurants, museums, parks and fairs
  • Painting, ceramics, wood crafts, cooking
  • Groups on art, history, music, and travel

FACILITY SERVICES

  • Curb-to-curb transportation
  • Specialized dementia care
  • Group and individual fitness programs
  • Modified sports such as bowling, volleyball, & shuffleboard
  • Crafts & Ceramics, watercolor & acrylic painting classes

SOCIAL SERVICES

Rainbow ADHC Social Workers coordinate with our nursing staff, therapeutic activities staff, and alternative therapists to help you attain or maintain your best possible physical, behavioral, and psychosocial well-being.

We also provide case management, advocacy to help you obtain services from other community agencies, and interpreter services when you receive mail or other correspondence from healthcare and government agencies.

Our social services include:

  • Assessments of social and emotional factors impeding health recovery
  • Counseling for long-range planning and decision-making
  • Community resource planning
  • Stress management and addiction counseling

REHABILITATION SERVICES

Rehabilitation Services at Rainbow ADHC promote your well-being, maintain and restore function, and manage deficits with activities of daily living.

  • Physical Therapist provides a full range of services which include, gait training, ultrasound, transfer training, home exercise program, prosthetic training, and training with durable medical equipment
  • Occupational Therapist provides evaluation activities of daily living training, muscle use re-education, perceptual motor training, fine motor coordination, neuro-developmental treatment, sensory treatment, orthodontics and splinting, and training with durable medical equipment
  • Speech Therapist provides  voice disorder treatment, speech articulation disorder treatment, dysphagia treatment, language disorder treatment, and oral rehabilitation
  • Respiratory Therapist provides treatment for respiratory-related challenges
  • Registered Dietitian provides counsel for healthy eating habits and proper diet

DEMENTIA AND ALZHEIMER’S PROGRAM

We provide a tailored package of support that not only helps our clients to enjoy the very best possible quality of life but which can also help to maintain and even improve physical or cognitive impairment.

  • Activities: we offer a full program of activities, tailored to meet the needs of people with Alzheimer’s. The right activities can stimulate normal neural activity, as well as provide considerable pleasure and stimulation to Alzheimer sufferer’s .Daily activities may include music, art, recreation, discussion and support groups.
  • Counseling: The center provides support services for people with dementia and their families. For example, they may offer guidance on outside resources and arrange for supportive care in the home
  • Special needs:  We can accommodate clients with the following special needs: someone who uses a wheelchair, who is hearing or visually impaired, wandering, incontinence, hallucinations, or speech difficulties.
